Power Mobility Coding 2006

The coding guideline is dated; however, it is historic as it began the end of easy billing for power mobility devices.

POWER MOBILITY DEVICE CODING GUIDELINES

BACKGROUND

Development of the new Power Mobility Device (PMD) Codes has been an extensive, iterative process. CMS and the SADMERC gratefully acknowledge industry participation and cooperation that was instrumental in the creation of this code set.

The codes presented are reflective of recommendations made by a Technical Expert Panel (TEP) commissioned by CMS and comprised of representatives from the technical, scientific, manufacturing, supplier, testing, and clinical aspects of the power mobility industry. The TEP recommendations were presented to CMS, the SADMERC, and the DME Program Safeguard Contractor (PSC) medical directors. This code set was adapted from those recommendations. Codes resulting from this collaborative effort allow placement of individual devices into performance-based categories.

All devices previously submitted for coding under the February 2005 parameters will be assigned a code. Code assignments are based on the performance testing data provided to the SADMERC for code assignment under the February 2005 code criteria. Assignment of a HCPCS code does not indicate coverage or payment for the item. This set of temporary codes is an interim step to allow code revision implementation in a step-wise and timely fashion.

Additional requirements were suggested by the TEP for safety, performance and durability testing and in response to CMS needs for incorporation of future technological advances. RESNA has clarified some test protocols, including a standard configuration for the device to be tested. From these recommendations, a new set of test parameters will be published. At least one year will be allowed for retesting. All devices will have to be retested in an independent testing facility according to those protocols to the new criteria. Timetables and test details will be published in a subsequent notice. Test results will be submitted to the SADMERC and a new coding verification process will be conducted. Policy and pricing changes will also be made as needed.

The February test standards were taken from the RESNA Vol. 1 & 2 standards. The referenced versions are: ANSI/RESNA WC/Vol. 1-1998 & ANSI/RESNA WC/Vol. 2-1998.

RESNA TESTING PARAMETERS USED FOR CODING

All the data submitted for code assignment based on the testing outlined in the February 2005 code set will be considered in the placement of devices for this application. Length, width, weight, weight of smallest components, speed, range, dynamic incline stability, obstacle height climb ability, ability to handle power devices, expandability of controller, ability to accept alternative input devices, seat types, and passing of the minimum of 200,000 fatigue cycles and 6,666 drop cycles will be used as the final determinants of code placement using TEP recommended division points between categories.

If new products are submitted for consideration for coding under this interim code set, the test protocols outlined in the February 2005 code set are to be followed. An updated copy will be on the SADMERC website for your reference. One change in the criteria form the February data is that only 200,000 fatigue cycles and 6,666 drop-cycles at the maximum patient weight are required. Testing to the February criteria may not be substituted for testing to the new protocols and criteria when they are published. Current devices will remain in the assigned codes unless reconfigured and retested. If a manufacturer has any questions, contact the SADMERC for advice.

II CODING GUIDELINES

There are six PWC Groups and two POV Groups. Groups are divided based on performance. Each group of PMDs will have sub-divisions based on patient weight capacity, seat type, portability, and/or power seating system capability. The full description of an individual PMD will require evaluation of all descriptors used.

The terms “Standard Duty”, “Heavy Duty”, etc., refer to weight capacity, not performance. For example, the term Group 3 Heavy Duty Power Wheelchairs denotes that the PWC has Group 3 performance characteristics and patient weight handling capacity between 301 and 450 pounds. A device is not required to carry all the weight listed in the class of devices, but must have a patient weight capacity within the range to be included. For example, a PMD that has a weight capacity of 400 pounds is coded as a Heavy Duty device.

For all Group 1 through Group 4 and Group 6 devices, the following equipment requirements apply. There is no separate billing for these features.

o Seat Width: Any width appropriate to weight group
o Seat Depth: Any depth appropriate to weight group
o Seat Height: Any height (adjustment requirements-none)
o Back Height: Any height (minimum back height requirement-none)
o Seat to Back Angle: Fixed or adjustable (adjustment requirements – none)

For all groups, manual semi-reclining backs are included in the base code and may not be billed separately.

III DEFINITIONS

1. Power Mobility Device (PMD) base codes include both integral frame and modular construction type power wheelchairs (PWCs) and Power Operated Vehicles (POVs).

2. Power Wheelchair – Chair-like battery powered mobility device for people with difficulty walking due to illness or disability, with integrated or modular seating system, electronic steering, and four or more wheel non-highway construction.

3. Power Operated Vehicle- Chair-like battery powered mobility device for people with difficulty walking due to illness or disability, with integrated seating system, tiller steering, and three or four-wheel non-highway construction.

4. Portable – A category of devices with lightweight construction or ability to disassemble into lightweight components that allows easy placement into a vehicle for use in a distant location.

5. Performance Testing is the term used to denote the RESNA based test parameters used to test PMDs. The PMD is expected to meet or exceed the listed performance and durability figures for the category in which it is to be used when tested. There is no requirement to test the PMD with all possible accessories.
6. Pediatric Power Wheelchair- Chair-like battery powered mobility device with capability for extensive growth through frame adjustments (not just seating), special features (i.e., seat to floor placement and/or standing capability, etc.) to address developmental issues, and uniquely sized for use with very small individuals with mobility restrictions.

7. Test Standards-Performance and durability acceptance criteria defined by ANSI/RESNA standard testing protocols.

8. Crash Testing-Denotes successful completion of WC-19 testing.

9. Top End Speed denotes the minimum speed acceptable for a given category of devices. It is to be determined by the RESNA test for maximum speed on a flat hard surface.

10. Range denotes the minimum distance acceptable for a given category of devices on a single charge of the batteries. It is to be determined by the appropriate RESNA test for range.

11. Obstacle Climb denotes vertical height of a solid obstruction that can be climbed using the standing and/or 0.5 meter run-up RESNA test.

12. Dynamic Stability Incline denotes the minimum degree of slope at which the PMD in the most common seating and positioning configuration(s) remains stable at the required patient weight capacity. If the PMD is stable at only one configuration, the PMD may have protective mechanisms that prevent climbing inclines in configurations that may be unstable.

13. Radius Pivot Turn – The distance required for the smallest turning radius of the PMD base. This measurement is equivalent to the “minimum turning radius” specified in the ANSI/RESNA bulletins.

14. PWC Basic Equipment Package-Each power wheelchair code is required to include all these items on initial issue (i.e., no separate billing/payment at the time of initial issue, unless otherwise noted):
a. Lap belt or safety belt (E0978)
b. Battery charger single mode (E2366)
c. Complete set of tires and casters any type (K0090, K0091, K0092, K0093, K0094, K0095, K0096, K0097, K0099)
d. Legrests. There is no separate billing/payment if fixed or swingaway detachable non-elevating legrests with/without calf pad (K0051, K0052, E0995) are provided. Elevating legrests may be billed separately.
e. Fixed/swingaway detachable footrests with/without angle adjustment footplate/platform (K0037, K0040, K0041, K0042, K0043, K0044, K0045, K0052)
f. Armrests. There is no separate billing/ payment if fixed/swingaway detachable non-adjustable armrests with arm pad (K0015, K0019, K0020) are provided. Adjustable height armrests may be billed separately.
g. Upholstery for seat and back of proper strength and type for patient weight capacity of the power wheelchair (E0981, E0982)
h. Weight specific components per patient weight capacity
i. Controller and Input Device. There is no separate billing/payment if a non-expandable controller and proportional input device (integrated or remote) is provided. If a code specifies an expandable controller as on option (but not a requirement) at the time of initial issue, it may be separately billed.

15. POV Basic Equipment Package-Each POV is to include all these items on initial issue (i.e., no separate billing/payment at the time of initial issue):
a. Battery or batteries required for operation
b. Battery charger, single mode
c. Weight appropriate upholstery and seating system
d. Tiller steering
e. Non-expandable controller with proportional response to input
f. Complete set of tires
g. All accessories needed for safe operation

16. Cross Brace Chair-A type of construction for a Power Wheelchair in which opposing rigid braces hinge on pivot points to allow the device to fold.

17. Power Options-Tilt, recline, elevating legrests, seat elevators, or standing systems that may be added to a PWC to accommodate a patient’s specific need for seating assistance.

18. No Power Options-A category of PWCs that is incapable of accommodating any power options.

19. Single Power Option-A category of PWCs with the capability to accept and operate only one power option at a time on the base. A PMD does not have to be able to accommodate all features to qualify for this code. For example, a power wheelchair that can only accommodate a power tilt could qualify for this code.

20. Multiple Power Option-A category of PWC with the capability to accept and operate more than one power option at a time on the base. A PWC does not have to accommodate all features from the defined list of power options to qualify for this code, but must be capable of having more than one power feature present and operational on the PWC at the same time.

21. Proportional Control Input Device-A device that transforms a user’s drive command (a physical action initiated by the wheelchair user) into a corresponding and comparative movement, both in direction and in speed, of the wheelchair. The input device shall be considered proportional if it allows for both a non-discrete directional command and a non-discrete speed command from a single drive command movement.

22. Non-Proportional Control Input Device- A device that transforms a user’s discrete drive command (a physical action initiated by the wheelchair user, such as activation of a switch) into perceptually discrete changes in the wheelchair’s speed, direction, or both.

23. Alternative Control Device- A device that transforms a user’s drive commands by physical actions initiated by the user to input control directions to a power wheelchair that replaces a standard proportional joystick. Includes head and chin arrays, sip and puff and other types of different input control devices.

24. Non-expandable Controller-An electronic system that controls the speed and direction of the power wheelchair drive mechanism. Only a standard proportional joystick can be used as the input device. This system may be in the form of an integral controller or a remotely placed controller. The non-expandable controller:

a. may have the ability to control up to 2 power seating actuators through the drive control (for example, seat elevator and single actuator power elevating legrests). (Note: Control of the power seating actuators though the Control Input Device would require the use of an additional component, E2310 or E2311.)
b. may allow for the incorporation of an attendant control.

25. Expandable Controller-An electronic system that is capable of accommodating one or more of the following additional functions:
a. other types of proportional input devices (e.g., mini or low-force joysticks, touchpads, chin control, head control, etc.)
b. non-proportional input devices (e.g., sip and puff, head array, etc.)
c. operate 3 or more powered seating actuators through the drive control. (Note: Control of the power seating actuators though the Control Input Device would require the use of an additional component, E2310 or E2311.)
An expandable controller may also be able to operate one or more of the following:
i. a separate display (i.e., for alternate control devices)
ii. other electronic devices (e.g., control of an augmentative speech device or computer through the chair’s drive control)
iii. an attendant control

26. Integral Control System-Non-expandable wheelchair control system where the joystick is housed in the same box as the controller. The entire unit is located and mounted near the hand of the user. A direct electrical connection is made from the Integral Control box to the batteries and motors through a high power wire harness.

27. Remotely Placed Controller-Non-expandable or expandable wheelchair control system where the joystick (or alternative control device) and the controller box are housed in separate locations. The joystick (or alternative control device) is connected to the controller through a low power wire harness. The separate controller connects directly to the motors and batteries through a high power wire harness.

28. Sling Seat/Back-Flexible cloth, vinyl, leather or equal material designed to serve as the support for buttocks or back of the user respectively. They may or may not have thin padding but are not intended to provide cushioning or positioning for the user.

29. Solid Seat/Back-Rigid metal or plastic material usually covered with cloth, vinyl, leather or equal material, with or without some padding material designed to serve as the support for the buttocks or back of the user respectively. They may or may not have thin padding but are not intended to provide cushioning or positioning for the user. PWCs with an automotive-style back and a solid seat pan are considered as a solid seat/back system, not a Captains Chair.

30. Captains Chair-A one or two-piece automotive-style seat with rigid frame, cushioning material in both seat and back sections, covered in cloth, vinyl, leather or equal as upholstery, and designed to serve as a complete seating, support, and cushioning system for the user. It may have armrests that can be fixed, swingaway, or detachable. It may or may not have a headrest, either integrated or separate.

31. Stadium Style Seat-A one or two piece stadium-style seat with rigid frame and cushioning material in both seat and back sections, covered in cloth, vinyl, leather or equal as upholstery, and designed to serve as a complete seating, support, and cushioning system for the user. It may have armrests that can be fixed, swingaway, or detachable. It will not have a headrest. Chairs with stadium style seats are billed using the Captains Chair codes.

32. Highway Use-Mobility devices that are powered and configured to operate legally on public streets.
